Clubs should note this request will be considered by the SW and is subject to approval.

Applications to change a fixture must be made by 19 June 2010

If reversing home and away venues remember to complete two forms - one for each date - ie one to move the home game to the away date and one to move to away date to the home date

PLEASE NOTE THIS IS NOT FOR NOTIFYING NEW DATES FOR GAMES POSTPONED DUE TO WEATHER OR RFU CUPS - THE SW WILL WRITE TO YOU WITH NEW DATES FOR THESE MATCHES
